Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Carrollton, OH. This role offers a flexible schedule, travel to patients' homes, and comprehensive training, with dedicated scheduling support and liability insurance options. Qualified NPs will review medical history, perform non-invasive assessments, document findings, and educate patients on wellness and preventive care.

Qualifications

• Master's Degree in Nursing (NP) and licensure.
• Active State NP license required.
• National board certification required.
• Current BLS certification required.
• 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.

Position Summary

Qualified Nurse Practitioners interested inthispositionwill have the opportunity to interact with members in a more intimate setting. Unlike a typical clinic visit, this role allows NPs to connect with patients on a more personal and effective level. Nurse Practitioners performing HRAs are not required to treat or prescribe in the home.
