Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Springfield, OH.

This role offers flexible scheduling, comprehensive home assessment training, and dedicated scheduling support to help you achieve a balanced, patient-centered practice outside the traditional clinic setting.

Qualifications

• Master’s Degree required or commensurate experience with NP license.
• Active state NP license required.
• Active national board certification.
• Current BLS certification.
• 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.

Education

Master’s Degree OR commensurate experience

Active State NP license

Active National board certification

Current BLS certification

12 months of experience as a paid advanced clinical provider (preferred)

Position Summary

Qualified Nurse Practitioners interested inthispositionwill have the opportunity to interact with members in a more intimate setting. Unlike a typical clinic visit, this role allows NPs to connect with patients on a more personal and effective level. Nurse Practitioners performing HRAs are not required to treat or prescribe in the home.

Nurse Practitioner Responsibilities
