Job summary

Hueman is actively hiring Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Cincinnati, OH. This role offers the flexibility to tailor a schedule and engage with patients in their homes, not in a clinic setting.

Qualified NPs will interact with members in a personal, non-clinic environment, performing comprehensive assessments while ensuring documentation accuracy. Training and support are provided.

Qualifications

• Master’s degree in nursing or commensurate experience.
• Active NP license in the state of practice.
• Active national board certification as NP.
• Current BLS certification.

Responsibilities

• Travel to patients’ homes to conduct HRAs.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document findings accurately and timely.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventive healthcare.

Position Summary

Qualified Nurse Practitioners interested inthispositionwill have the opportunity to interact with members in a more intimate setting. Unlike a typical clinic visit, this role allows NPs to connect with patients on a more personal and effective level. Nurse Practitioners performing HRAs are not required to treat or prescribe in the home.

Nurse Practitioner Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.

Nurse Practitioner Qualifications and Experience Requirements:

• Master’s Degree OR commensurate experience and satisfactory completion of NP license
• Active State NP license
• Active National board certification
• Current BLS certification
• 12 months of experience as a paid advanced clinical provider is preferred but not required
