Hueman is hiring Nurse Practitioners to conduct in-home Health Risk Assessments for Medicare and Medicaid members in and around Springfield, OH. This role offers flexible scheduling and the ability to customize your workload while maintaining professional autonomy.

Practitioners will travel to patients' homes to perform assessments, document findings, and provide basic wellness education. NP licenses, national certification, and BLS are required; 12 months of paid clinical provider experience is

Qualifications

• Masters Degree in Nursing or commensurate experience with NP license.
• Active State NP license required.
• Active National board certification required.
• Current BLS certification.
• 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to and from patients' homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventive healthcare.

Position Summary

Qualified Nurse Practitioners interested inthispositionwill have the opportunity to interact with members in a more intimate setting. Unlike a typical clinic visit, this role allows NPs to connect with patients on a more personal and effective level. Nurse Practitioners performing HRAs are not required to treat or prescribe in the home.

Nurse Practitioner Responsibilities
