About the Position:

During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.

Responsibilities:

What to Expect:

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education, referrals, follow-up, and documentation according to Matrix guidelines and protocols.
• Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and other medical personnel as appropriate.
• Collaborate with Primary Care Physician (PCP) on patient education and follow-up.
• Provide services in various venues, including home visits and skilled facility visits.

Nurse Practitioner - PRN - Home Risk Assessments

Type: PRN - Flexible - Nurse Practitioner

PAY: Earn from $2,200 up to $11,000 per month (based on an average of 20-100 completed visits per month, at $110 per assessment). Compensation varies with availability and assessment completion.

Location: Home, SNF, and other community environments in Luray, Virginia

Hours: Days, evenings, nights, weekends, with flexible scheduling

Benefits: PRN providers are non-benefited except for 401(k) and limited voluntary benefits.

Mileage reimbursement plan included.

Qualifications:

What is Required:

• Master’s Degree or equivalent experience with successful NP licensure
• Current RN and NP licensure in the state of practice, including prescription authority or ability to obtain it
• Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
• Current BLS, ACLS, or CPR certification
• Preferably 1 year of experience as a Nurse Practitioner; new grads are encouraged to apply
• Strong computer skills and familiarity with medical record software
• Excellent verbal and written communication skills
• Flexibility and comfort with frequent change

Travel Requirements:

• Travel required to meet patients at their locations
• Valid driver’s license, ability to drive, and proof of adequate auto insurance

Job Type: Clinical Nurse Practitioner (NP), PRN, flexible hours, no on-call, no evening or weekend hours unless desired. Work from home opportunities available.
