Home Risk Assessments

Position Details

During visits lasting up to one hour, providers review health status, medical history, medications, social environment, and risks. This comprehensive assessment enhances care quality and helps address care gaps efficiently.

Responsibilities

• Conduct adult/geriatric assessments including history, diagnosis, treatment, education, referrals, and documentation
• Collaborate with physicians, case managers, social workers, and family members
• Coordinate with Primary Care Physician for patient follow-up and education
• Provide services in home, skilled facilities, and community settings

Position Details

• Type: PRN - flexible schedule
• Pay: From $2,200 to $11,000/month based on visits and assessments
• Location: Onanock, VA, with travel to patient homes and facilities
• Hours: Days, evenings, nights, weekends; flexible scheduling
• Benefits: Limited benefits, including 401(k) and voluntary options; mileage reimbursement

Qualifications

• Master’s degree or equivalent experience and NP licensure
• RN and NP licensure with prescriptive authority in practice state
• Board certification (AANP, ANCC, or AACCN)
• Current BLS, ACLS, or CPR certification
• 1+ year NP experience preferred; new grads welcome
• Strong computer skills and familiarity with medical records software
• Excellent communication skills
• Flexibility with change

Travel Requirements

• Must be able to travel to patient locations
• Valid driver’s license and insurance required
