Matrix Medical Network is seeking compassionate and dedicated Nurse Practitioners to join our team for home risk assessments. We provide an excellent opportunity for new graduates and experienced NPs to make a meaningful impact on patient care in Bronx, NY, and surrounding areas.

Position Overview

This PRN role involves conducting comprehensive health assessments during home visits, lasting up to one hour, to evaluate health status, medication adherence, social factors, and other risks. These visits help close care gaps and improve patient outcomes.

Responsibilities

• Perform adult and geriatric assessments, including medical history, diagnosis, treatment, and health education.
• Collaborate with physicians, case managers, and caregivers to coordinate care.
• Follow up with patients and provide education in various settings, including home visits and skilled facilities.

Qualifications

• Master’s Degree or equivalent experience with NP licensure.
• Current RN and NP licensure in New York, with prescriptive authority.
• Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
• Current BLS, ACLS, or CPR certification.
• Minimum of 1 year NP experience preferred.

Details

• Pay: From $2,300 to $11,500 per month based on assessments completed.
• Location: Bronx, NY, with travel to patients’ homes and community sites.
• Schedule: Flexible hours including days, evenings, nights, and weekends.
• Benefits: PRN providers are non-benefited except for 401(k) and voluntary benefits; mileage reimbursement included.

Travel Requirements

Must have a valid driver’s license, automobile insurance, and ability to travel to patient locations.
