Position Overview

As a Nurse Practitioner, you'll conduct comprehensive health assessments during home visits, reviewing health, medical history, medication adherence, social factors, and risks. These visits help improve care quality and close care gaps efficiently.

Responsibilities

• Perform adult/geriatric assessments, including medical history, diagnosis, treatment, education, and referrals.
• Collaborate with healthcare team members and primary care physicians.
• Provide services in various settings, including home and skilled facilities.

Position Details

• Type: PRN, flexible hours, day/evening/nights/weekends.
• Location: Home, SNF, community environments in Erie, PA.
• Compensation: Earn from $2,000 to $10,000/month, based on assessments completed.
• Benefits: Limited benefits, including 401(k) and mileage reimbursement.
