Job summary

Premise Health Inc. seeks a Nurse Practitioner (PRN) to provide primary and occupational health care for Kwik Trip employees at clinics in DeForest and Madison, WI. Roles include clinical exams, care plans, and referrals as needed.

The position supports occasional temporary full-time coverage in Jan-Feb 2027; clinic hours vary by day. Competitive pay and access to Premise wellness and benefits are offered to eligible staff.

Qualifications

• Active RN license in the state of practice.
• Nurse Practitioner license/certification with prescriptive authority.
• DEA license for controlled substances where applicable.
• Current national certification (ANCC or AANP) required.
• Master’s Degree in Science of Nursing.
• BLS/ACLS certification per contract scope.
• Minimum of three years NP clinical experience in ambulatory/occupational health or related setting.
• Occupational health certification (COHN/COHN-S/CM) preferred.

Responsibilities

• Perform physical exams and preventive health measures per guidelines.
• Conduct fitness for duty and return-to-work exams as requested.
• Provide clinically appropriate care for injured workers using evidence-based guidelines.
• Perform procedures such as injections, suturing, wound care, and infection management.
• Develop care plans, document progress, and ensure continuity of care.
• Educate patients on treatment plans and health maintenance.
• Order and interpret diagnostic tests; coordinate with physicians.
• Collaborate with care teams and refer to specialists as needed.
• Prescribe or recommend treatments and therapies.
• Support and coach nursing staff on best practices.

As a Nurse Practitioner - PRN you'll provide care to client employees and their dependents of Kwik Trip in our Health Center located in DeForest & Madison, WI.

Schedule

• PRN as Needed During Clinic Hours
• Clinic Hours:
  • Mon & Tue 6:30 am - 4:30 pm
  • Wed & Thu 11:00 am - 7:00 pm
  • Fri 8:00 am - 12:00 pm
• Provide Primary Care and Occupational Health services at clinics in DeForestand Madison
• Potential opportunity for temporary full-time coverage in January & February 2027

What You’ll Do

• Conducts physical exams and preventive health measures per guidelines
• Conducts fitness for duty and return to work physical exams as requested.
• Provides medically appropriate care to injured workers using evidence-based guidelines, expedites the return to meaningful work, and collaborates with the TPA and client to quarterback case management.
• Performs procedures such as injections, suturing, wound care, and infection management
• Develops patient care plans, records progress, and ensures continuity of care
• Educates patients on treatment plans, health maintenance, and development
• Orders and interprets diagnostic tests to assess clinical conditions
• Collaborates with physicians and healthcare teams for comprehensive care
• Prescribes or recommends treatments, including medications and therapies
• Refers patients to specialists or physicians as needed
• Maintains patient health records ensure accurate and up-to-date documentation and records
• Educates and coaches nursing staff on best practices
• Performs other duties as assigned

What You’ll Bring

• Maintains state RN licenses/certificates to practice
• Maintains Nurse Practitioner licenses/certificates to practice
• Maintains State Prescriptive Authority (if applicable)
• Federal DEA license (matching each state of practice as applicable)
• Current national certification (ANCC or AANP required)
• Master’s Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required)
• Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services
• At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting
• Certification in occupational health (COHN/COHN-S/COHN-S/CM) preferred.
