Qualifications

Responsibilities

• Deliver evidence-based primary and family care, including assessment, diagnosis, treatment and management of acute and chronic conditions.
• Provide both in-person and virtual visits from the clinic to support patient outcomes.
• Perform diagnostic testing and preventive counseling, and coordinate care with other clinicians.
• Engage patients in wellness services, chronic disease management and preventive screenings.
• Handle essential clinic operations including inventory, opening/closing, patient follow-up, insurance verification and payments.
