Job summary

CVS Health is seeking a Full-Time Regional Provider to deliver high-quality primary and family care across Phoenix, Scottsdale, Tempe, Gilbert, Mesa and Chandler HealthHub locations. This clinic-based role includes in-person and virtual visits, with rotating 3- and 4-day workweeks and weekends.

Providers will deliver evidence-based care, vaccines, and chronic disease management while coordinating with a multidisciplinary team. The position offers competitive pay and comprehensive benefits.

Qualifications

• One year NP experience preferred; new graduates may be considered
• Active, unrestricted Family Nurse Practitioner license in good standing
• Obtain and maintain multi-state licensure, based on business needs
• Ability to provide holistic, evidence-based care including contraception and STI prevention
• Ability to order/administer vaccines and injections (COVID-19, influenza, TB testing, Depo-Provera)
• DOT certification within 30 days of employment
• Active Basic Life Support (BLS) certification
• Ability to pass a respirator FIT test
• Physical presence at assigned clinic required
• Prior PA experience preferred; qualified new graduates considered
• Active, unrestricted state PA license (or eligibility to obtain upon hire)
• DOT certification exam completed within 30 days of employment
• Active BLS certification
• Ability to pass a respirator FIT test
• Physical presence at assigned clinic required

Responsibilities

• Deliver evidence-based primary and family care, including diagnosis and management
• Provide in-person and virtual visits from the clinic
• Perform diagnostic testing, preventive counseling, and treatment
• Engage patients in wellness services and chronic disease management
• Complete clinic operations: inventory, opening/closing, follow-up, insurance verification, billing
• Support a safe and efficient care environment and timely appointments
