Job summary

U.S. Department of Veterans Affairs is seeking a full‑time Advanced Practice Nurse (APN) Certified Nurse Practitioner to deliver primary care in Salina, Kansas.

The NP will manage a panel of Veterans, emphasize health promotion and disease prevention, and coordinate care with the Primary Care Team. Qualifications

Duties

• The Robert J Dole VA Medical Center (VAMC) is seeking a full-time Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) that will be responsible for providing Primary Care services to Veterans of the Robert J
• Dole VAMC in Salina - Kansas! The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Primary Care Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission
• The NP provides advanced clinical practice - consultation - and management
• The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients
• Major duties and responsibilities of the position include but not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
• Develops and implements an individualized plan to achieve optimal population health and disease management
• Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members
• Orders and interprets labs - tests - and diagnostic studies
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals and consults to specialty services
• Provides education to patients and their families
• Assess and adjusts the plan of care as needed to achieve optimal patient outcomes
