Job summary

U.S. Department of Veterans Affairs is seeking an Advanced Practice Nurse (NP) in Primary Care within the VA Midwest Health Care Network.

The NP will provide a full range of services emphasizing quality, timeliness, and the VA mission, managing an adult patient panel with emphasis on health promotion and disease prevention. The role includes leadership within the Primary Care Team, ordering tests, prescribing interventions, initiating referrals, and educating patients and families.

Responsibilities

  • Independent manages a panel of patients with acute and chronic health conditions with emphasis on health promotion and disease prevention using evidence-based practice
  • Develops and implements an individualized plan to achieve optimal population health and disease management
  • Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members
  • Orders and interprets labs, tests, and diagnostic studies
  • Prescribes pharmacologic and non-pharmacologic interventions
  • Initiates referrals and consults to specialty services
  • Provides education to patients and their families
  • Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
  • Provider may be required to travel to the CBOCs or provide gap coverage and to be a designated women's provider with VHA training

Job description

Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Duties
  • The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers
  • This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest

  • The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Primary Care Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission
  • The NP provides advanced clinical practice - consultation - and management
