Job summary

Oak Street Health in Oak Park, IL seeks a Nurse Practitioner to deliver effective, equitable primary care to adults on Medicare. You will work with a collaborative care team to provide longitudinal care, conduct visits, and coordinate with specialists to keep patients healthy and out of the hospital.

Responsibilities include annual wellness visits, care coordination, patient education, and contributing to population health initiatives.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program.
• National certification in at least one of: Family Nurse Practitioner, Adult-Gerontology Primary Care NP, Adult NP, Gerontological NP.
• Active, non-probationary state NP license and Active DEA license.
• US Work Authorization required.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership, in coordination with the Care Team.
• Assisting the care team with phone triage and outreach.
• Educating patients on their health conditions, care plans, and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties, as assigned.
• This role reports to the Center Medical Director and works closely with operational leadership.
