Qualifications

• Active Nurse Practitioner license in the state of practice.
• Board Certified Nurse Practitioner (FNP, AGNP, ANP, or equivalent).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and relationship-building abilities.
• Ability to work independently while collaborating effectively with facility teams.

Responsibilities

• Serve as the primary care provider for assigned residents.
• Perform initial assessments and routine follow-up visits.
• Manage chronic and acute medical conditions.
• Round throughout the facility daily and maintain a visible clinical presence.
• Collaborate closely with nursing staff, therapists, social workers, and administration.
• Respond to resident condition changes and provide timely interventions.
• Participate in care plan meetings and interdisciplinary discussions.
• Communicate with residents and families regarding care plans and medical concerns.
• Help reduce avoidable hospitalizations through proactive clinical management.
• Maintain accurate and compliant medical documentation.
• Support facility quality improvement and regulatory readiness initiatives.
