Job summary

CareMed Health Solutions is seeking a Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities in Colorado. You will manage acute changes, participate in advanced care planning, and coordinate with an interdisciplinary team to deliver optimal care.

Responsibilities include prescribing medications per state law, ordering diagnostics, and communicating updates to facility staff and primary physicians.

Qualifications

• Current, unrestricted NP license in Colorado.
• Board Certification as NP.
• DEA licensure is required.
• Clinical background in adult, family or geriatric care.
• Experience in long-term care settings.
• Ability to work with an EMR.
• Ability to manage patients with complex chronic conditions.
• Ability to collaborate within an interdisciplinary care team.
• Psych experience is a plus.

Responsibilities

• Provide primary and preventive care for long-term care residents in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Perform advanced care planning and chronic care management.
• Prescribe medications, including controlled substances per Colorado law.
• Order and follow up on treatments and diagnostics.
• Communicate changes in condition with nursing home staff and care plan.
• Collaborate with the primary care physician.
