HarmonyCares is seeking a Nurse Practitioner with experience in home health, geriatrics, and primary care to provide patient-centered care in patients’ homes Monday through Friday.

The role emphasizes taking time to address each patient’s needs and offers an opportunity to contribute within an accountable care model, with a competitive base salary and CME/license support.

Qualifications

• Licensed as a NP in the state where HarmonyCares Medical Group office located.
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Directly related experience in home health, geriatrics, hospice and primary care.
• Practice medicine with a focus on patient care, not volume.
