Responsibilities:
• Facilitates entry of the patient into the health care system, by assessing the primary health care needs, including eliciting comprehensive health histories, performing physical examinations, ordering and/or performing pertinent diagnostic tests, analyzing collected data to determine client health status and formulating a problem list (diagnosis.)
• Provides coordinated, continuous and culturally competent primary health care to clients that span the health continuum through mutual goal setting and planning delivery of care
• Perform physical examinations, order laboratory tests, and interpret information collected in comprehensive health assessment to identify and assess patient's clinical concerns and health care needs.
• Provide follow-up information to patients about abnormal test results and necessary referrals, and follow mandatory reporting requirements.
• Prescribes a variety of treatment modalities to meet health and illness needs
• Collaborates with the Director of Outreach and Prevention and Medical Director, nurse practitioner, community health coaches and other clinic staff to assure that health teaching and counseling is provided
• Meets average monthly provider productivity standards including but not limited to number of visits
• Manages and evaluates the plan of care, process and outcome and then modifies the plan as needed.
• Collaborates with other multidisciplinary professionals and community agencies to provide high quality care
• Coordinates services in conjunction with other team members to meet patient’s primary health care needs and to ensure the continuity of care.
• Diagnosis, treatment, and prevention of sexually transmitted infections, HIV and related conditions.
• Provide general health supervision, counseling and education to patients regarding health promotion maintenance and individual high-risk conditions.
• Refer deviations from normal treatment and diagnosis protocols to the medical director of the clinic or the appropriate health care professional.
• Prescribes and administer medication for treatment of diagnosed conditions or the prevention thereof.
• Complete prior authorizations and appeals as necessary
• Complete home visits as necessary
• Maintain up-to-date knowledge of disease progression and relevant therapies
• Utilizes electronic medical records system to document patient care including update patient status.Charts must be closed within 24 hours or by COB next business day.
• Maintain confidential records in keeping with all applicable ethical guidelines and legal requirements.
• Assist in routine review, and revision of organizational policies and procedures, as applicable.
• Correct deficits raised by audits to ensure quality medical standards.
• Knowledge of and compliance with Tennessee Department of Health and local legal mandates.
• Assist in development and implementation of clinical services expansion including, but not limited to, primary care, HCV, and medication-assisted therapy for opiate use disorder.
• Assist in the implementation of third-party billing.
• Assist in the hiring and supervision of an expanding clinical services team
• Work as assigned, including weekends and evening hours and at other clinic sites as needed.
• Duties and responsibilities may be added, deleted or changed at any time at the discretion of management, formally or informally, either verbally or in writing. Assist in providing accurate, complete and timely data entry and reporting of all relevant program and consumer data into CAREware and/or other reporting systems as requested by supervisor.
• Submit quarterly/monthly data for reports by the 5th day of each program quarter/month as assigned.
• Submit data for annual reports by the 5th day of the month following the end of the program year as assigned.
Education and Training:
• Graduation from an accredited college or university, or an equivalent program as determined by the TN Board of Nursing, with a Bachelor's Degree in Nursing and a Master's Degree in Public Health, Community Health Nursing or other appropriate nursing specialty; and successful completion of a Nurse Practitioner Program.
• Three (3) years of full-time equivalent post-licensure experience as a Registered Nurse; two (2) years of these 3 years must have been in patient evaluation, diagnosis and treatment and one (1) year of these 2 years must have been in community health nursing.
• Experience working with persons with HIV/AIDS and/or other issues including sexual health, reproductive health, drug use, homelessness, mental illness
• Active NPI and TennCare provider ID are required
• Medicaid provider ID for MS and AR preferred
• Must have a valid State Driver’s License, provide proof of automobile insurance coverage to employer at all times and have an automobile to be used daily to perform job duties.
• Must have a working knowledge of MS Windows and MS office suite with an emphasis on MS Word, MS Excel, and MS Outlook.
• Proven ability to accomplish duties
• Experience with Netsuite, NBS reporting, Evaluation Web, RedCap, CareWare (preferred)
• Good strategic/critical thinker, self-starter, goal-oriented
• Excellent verbal and written communications.
• Ability to operate with purpose, urgency and accuracy in a fast-paced, deadline-driven environment -- with particular attention to detail and organization
• Strong relationship-builder both with team and clients(internal and external)

This position performs tasks that may involve exposure to blood, body fluids, or tissues. Training will be provided per Standard Operating Procedure for your area of operation. All client employees are offered the opportunity to receive the Hepatitis B vaccination series.
