Job summary

Altea Healthcare is seeking a Certified Nurse Practitioner to float between Overton and Tyler, TX, in a post-acute and skilled nursing setting. You will diagnose, treat, and guide patients toward better health as a key clinical provider in a busy SNF, with a focus on geriatrics and primary care.

The role offers a base salary range of $100,000–$135,000 per year, plus uncapped bonus potential, full benefits, and strong mentorship for both new grads and experienced providers.

Qualifications

• NP license or eligibility to obtain.
• Current DEA registration or willingness to obtain prior to start date.
• All experience levels welcome — training & mentorship available.

Responsibilities

• Performing physical exams and reviewing medical histories.
• Ordering and interpreting diagnostic tests (labs, imaging, etc.).
• Diagnosing and managing acute and chronic conditions.
• Prescribing medications and creating treatment plans.
• Collaborating with physicians, nurses, and facility staff.
• Educating patients and families on health conditions and preventive care.
• Documenting patient care accurately and efficiently.
