Nurse Practitioner – In-Home Acute Care
Schedule & Compensation:
Full-time: $176,800–$197,600/year (based on experience) + signing bonus
Part-time: $85–$95/hour
Includes evenings, weekends, and holidays | Shifts range from 8 to 12 hours

About the Role
We are hiring Nurse Practitioners to deliver acute-level care in patients’ homes throughout the Greater NYC area. This model blends the responsiveness of urgent care with the comfort and impact of house calls. Providers are supported by a dedicated interdisciplinary team and backed by diagnostic tools and remote physician oversight.
This is a unique opportunity for Nurse Practitioners who are confident practicing independently and passionate about transforming care for medically complex and underserved populations.
Key Responsibilities
Respond to unscheduled referrals and perform urgent, high-acuity in-home visits
Conduct clinical assessments and focused physical exams
Administer treatments such as IV fluids, wound care, splinting, and injections
Order and interpret labs, EKGs, and bedside diagnostics (e.g., point-of-care ultrasound)
Coordinate with referring providers and care managers to ensure continuity of care
Educate patients and caregivers with a palliative, patient-centered approach
Provide telehealth-based follow-up over 7–30 days, depending on acuity
Qualifications
Active NYS Nurse Practitioner license
Current BLS certification (ACLS preferred)
Minimum of 2 years’ experience in emergency medicine, urgent care, hospital medicine, or similar high-acuity setting
Ability to work independently in a fast-paced, field-based role
Strong clinical judgment, communication, and adaptability
Availability for a mix of shifts, including evenings, weekends, and holidays
