Overview of the Role:
The Alignment Jump Start Assessment (JSA) program is a physician-led and advanced practice clinician-driven initiative designed to support patients with complex and chronic care needs. As a JSA Advanced Practice Clinician (APC), you will work within an interdisciplinary team—including physicians, nurse practitioners, physician assistants, case managers, and other healthcare team members—to ensure proper delivery of clinical and home-based patient care. Your responsibilities include assessing, developing, and coordinating care options to promote quality outcomes while ensuring cost-effective care in compliance with company policies and regulations. Members are typically aged 65 and over, with visits lasting 45 to 60 minutes depending on the visit type.

Pay Range: $70.00 - $90.00 per hour.
