Overview of the Role:

The Alignment Jump Start Assessment (JSA) program is a physician-led and advanced practice clinician-driven initiative designed to support patients with complex and chronic care needs. As a JSA Advanced Practice Clinician (APC), you will work within an interdisciplinary team—including physicians, nurse practitioners, physician assistants, case managers, and other healthcare team members—to ensure proper delivery of clinical and home-based patient care. Your responsibilities include assessing, developing, and coordinating care options to promote quality outcomes while ensuring cost-effective care in compliance with company policies and regulations. Members are typically aged 65 and over, with visits lasting 45 to 60 minutes depending on the visit type.

Responsibilities:

• Conduct in-home assessments, including obtaining comprehensive history, physical exams, medication reviews, and cognitive/depression/safety screenings.
• Engage in end-of-life care discussions as needed.
• Collect laboratory specimens in the home setting when appropriate.
• Identify diagnoses for active medical management focusing on chronic disease management.
• Communicate assessment findings to the PCP, highlighting potential care gaps.
• Provide education to patients, families, and caregivers, emphasizing monitoring and follow-up.
• Refer patients to appropriate community resources.
• Comply with HIPAA regulations and maintain PHI security.

Required Skills and Experience:

• Master’s degree from an accredited NP or PA program.
• Preferably one year of clinical or home care experience.
• Previous EMR experience preferred.
• Active California Nurse Practitioner or Physician Assistant license.
• Active RN license and Furnishing number (NP only).
• Board certification for Nurse Practitioners or Physician Assistants.
• NPI number, DEA registration, and current BLS certification.
• Valid California driver’s license and insurance.
• Experience caring for older adult (geriatric) patients preferred.
• Excellent organizational, administrative, and communication skills.

Essential Physical Functions:

The physical requirements include talking, hearing, standing, walking, sitting, handling objects, reaching, and lifting up to 20 pounds. Vision abilities include close vision and focus adjustment. Reasonable accommodations may be provided for disabilities.

Pay Range: $70.00 - $90.00 per hour.
