Job summary

Carroll Health and Rehab is seeking a Pulmonary Nurse Practitioner or Physician Assistant to provide comprehensive pulmonary care in a post-acute setting. The role involves collaborating with physicians, nursing staff, and allied professionals to optimize respiratory outcomes.

You will assess, diagnose, and manage COPD, asthma, pneumonia, and respiratory failure, while developing individualized treatment plans and maintaining EMR documentation.

Qualifications

  • 3 years nursing experience in a clinical setting.
  • 1 year PA/NP experience in pulmonary care.
  • Active and unrestricted NP or PA license with NCCPA certification.
  • Master’s Degree in Nursing or Physician Assistant program required.
  • BLS/BCLS certification preferred.

Responsibilities

  • Conduct pulmonary assessments for post-acute care patients.
  • Diagnose and manage COPD, asthma, pneumonia, and respiratory failure.
  • Develop individualized treatment plans and monitor progress.
  • Collaborate with interdisciplinary teams and educate staff.

Job Summary:

Experience:
  • Nursing: 3 years (Required)
  • PA or NP: 1 year (Required)
License/Certification:

Job Responsibilities
  • Conduct pulmonary assessments for patients in post-acute care settings
  • Evaluate, diagnose, and manage pulmonary conditions including COPD, asthma, pneumonia, and respiratory failure
  • Develop and implement individualized treatment plans
  • Collaborate with interdisciplinary teams
  • Educate patients, families, and facility staff on respiratory care and treatment plans
  • Monitor patient progress and adjust care plans as needed
  • Maintain accurate and timely EMR documentation
  • Support initiatives focused on reducing hospitalizations and Return-to-Hospital (RTH) events
Qualifications
  • Preferred a minimum of 6 months of experience in post-acute care setting
  • Preferred experience in Pulmonary: training provided
  • Active and unrestricted state Nurse Practitioner license or Active PA license and NCCPA certification required
  • Master’s Degree in Nursing from an accredited Nurse Practitioner program or Master’s Degree from an accredited Physician Assistant program required.
  • CPR BCLS/BLS certified
  • Strong communication, organizational, and teamwork skills
  • Ability to work independently in the clinical environment
