Theoria Medical is seeking a full-time Advanced Practice Provider in Ayer, MA to serve local skilled nursing facilities. Flexible scheduling and autonomy are offered, supported by a collaborative clinical team and modern technology to streamline care.

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounds, prescribe medications/treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on-site attendance for scheduled shifts.

Your Qualifications

Advanced Practice Providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants).
