Job summary

Theoria Medical seeks an Advanced Practice Provider in Ayer, MA to deliver patient-centered care in skilled nursing facilities. You will round daily, prescribe treatments, and collaborate with a clinical team using intuitive technology and an iPad-based workflow.

Qualifications

• Requires active U.S. license and board certification for AP roles.
• Specialties include internal medicine, family medicine, geriatrics, or similar.

What You’ll Do

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.

Your Qualifications

Advanced Practice Providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
