Carolina Senior Care in Lexington, NC is hiring a Physician Assistant or Nurse Practitioner to provide primary care under the Medical Director, perform assessments, develop care plans, and collaborate with the IDT to care for frail elders.

The role requires a Bachelor's Degree, North Carolina license, CPR/BCLS, and experience with elderly populations; preferred geriatric experience; 5 days a week, 8 AM to 4 PM; no weekends.

Qualifications

• Bachelor’s degree required; MD/DO or PA/NP pathways accepted.
• Current North Carolina license to practice medicine or advanced practice.
• Experience with frail or elderly population strongly preferred.
• Prescriptive authority for NP preferred; board certification is a plus.

Responsibilities

• Provide primary care under Medical Director supervision.
• Perform assessments, develop and adjust care plans.
• Educate participants and caregivers; coordinate with IDT.
• Manage acute and long-term care, discharge planning.
• Maintain records, handle coding, and order labs.

Physician Assistant/Nurse Practitioner, 5 Days a Week, 8 AM to 4 PM

Position Summary: "Empowering Elders, Enriching Lives"

Under the Medical Director's supervision, the Doctor/MD, PA or NP provides primary care, performs assessments, develops care plans, treats participant complaints, educates participants and caregivers, and collaborates with the Interdisciplinary Team (IDT) to care for frail elders according to Every Age PACE standard.

Essential Duties and Responsibilities:

Team Collaboration: Participates in meetings, communicates changes, and collaborates on care plans.

Participant Care: Conducts assessments, exams, and integrates care plans.

Acute and Long-term Care: Manages illnesses, nursing home care, and discharge planning.

Administrative Tasks: Maintains records, handles coding, orders lab tests, and ensures safety.

Bachelor’s Degree (Required)

• Doctor/MD: Doctorate Degree (Includes PhDs): MD or DO
  • Board Certification – Internal Medicine or Family Practice
  • Board Certification – Geriatrics preferred
  • Current North Carolina physician license
  • Current BCLS
• North Carolina certification and DEA Registration
• Physician Assistant: Graduate of certified or accredited Physician Assistant Program
• Nurse Practitioner: RN license and Nurse practitioner license with prescriptive authority preferred.
• CPR certification (Required)

A minimum of one year’s experience in working with the frail or elderly population (Required)

Two years’ experience as MD/Physician Assistant/Nurse Practitioner in a geriatric setting (Preferred)
