Cornerstone Clinical Partners is seeking a Primary Care Nurse Practitioner/Physician’s Assistant to provide on-site care to residents in long‑term care facilities. This part‑time role emphasizes meaningful patient relationships and a collaborative care environment.

Qualified providers have NP/PA licenses and NC licensure, with at least 6 months in long‑term care. You’ll diagnose, treat, educate, and coordinate with facility teams while supporting elder‑care goals in a structured setting.

• Minimum 6 months of experience in long‑term care or a skilled nursing facility
• Strong communication, clinical, and critical thinking skills
• Passion for elder care and person‑centered healthcare
• Prolonged standing/walking. Occasional lifting up to 20 lbs. Computer work. Lifting/moving patients. Prolonged sitting.
