Job summary

UW Health in Madison, WI, on-site, is seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our care teams in psychiatric and AODA services.

You will conduct assessments, see patients for new and follow-up care, including medications and psychotherapy, and coordinate care for both acute and chronic health needs. Minimum licensure requirements include Wisconsin RN/APNP and PA certifications. DEA license is required or can be obtained on hire.

Qualifications

• Master’s or Doctorate degree from accredited Nursing program.
• Bachelor’s or Master’s degree from accredited PA program.
• Active RN and APNP license in Wisconsin and board certification.
• Active PA license in Wisconsin and PA board certification.
• Active DEA license or ability to apply for such license prior to hire.

Work Schedule:
Full-time, Monday through Friday, 8:00 am- 5:00 pm. You will work at 1102 S. Park Street in Madison, WI.

We are seeking a Nurse Practitioner or Physician Assistant to:

• Conduct psychiatric and AODA assessments of patients in an inpatient and outpatient setting.
• See patients as needed for new and follow-up psychiatric care, AODA treatment, including medications and psychotherapy.
• Be responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination for patients with acute and chronic health needs.
• Master’s or Doctorate’s degree from accredited Nursing Program
• Bachelor’s or Master’s degree from accredited PA Program

Licenses and Certifications:
Minimum –
If Nurse Practitioner:

• Active RN and APNP license in State of Wisconsin AND
• Active board certification as a NP in Psychiatric-Mental Health.
• Active PA license in State of Wisconsin AND
• Active PA board certification

All Nurse Practitioner and Physician Assistant applicants must have:
