Salud Family Health- in Trinidad, CO, is seeking a Full-Time Nurse Practitioner or Physician Assistant to join our Medical Provider team. You will perform routine clinic tasks, take patient histories, and conduct physical assessments within a collaborative, team-based care model.

We offer a competitive starting salary of USD 118,100 to USD 151,850 annually, relocation assistance up to USD 5,000, comprehensive health benefits, retirement plans, and generous paid time off.

Qualifications

• Must have graduated from an accredited Nurse Practitioner or Physician Assistant program.
• Unrestricted Colorado NP/PA license in good standing.
• 3 years of clinical experience preferred.
• Spanish-speaking is strongly preferred.

• Perform routine clinic operations, take histories, and conduct physical assessments.
• Collaborate in a team-based care model to coordinate patient care.

Job Description

We're seekinga Full-Time Nurse Practitioner or Physician Assistant with Salud Family Health in Trinidad, CO.

At Salud, our Physician Assistants & Nurse Practitioners approach their tasks in a team-based care approach that supports patients and their families in self-management, self-efficacy, and behavior change. As a Physician Assistant/Nurse Practitioner at Salud, you will perform routine clinic operations, take histories, and make physical assessments, all the while adhering to the spirit of the SALUD mission statement.

• Starting Annual Salary - $118,100 to $151,850

Qualifications

Education:

• Must have graduated or been graduating from an accredited Nurse Practitioner or Physician Assistant program

Licensure:

• Possess an unrestricted Physician Assistant license or Nurse Practitioner license in good standing in the State of Colorado

Knowledge, Skills, and Abilities:

• The ability to speak Spanish is stronglypreferred


• Sensitivity to low-income and ethnic minority communities is a must

Experience:

• 3 years of experience in a clinical setting as a Nurse Practitioner / PhysicianAssistant preferred
