Job summary

Altais-Medical-Group is seeking a CA-licensed Nurse Practitioner with at least 1 year of primary care experience. The role involves comprehensive health assessments, medication reviews, and documentation for STAR/HCC measures.

You will document in patient charts on the day of visits and collaborate with families to optimize care. The position offers a base salary of $135,000–$155,000 per year plus a sign-on bonus, with a robust benefits package including medical, vision, dental, 401k, flexible

Qualifications

• CA licensed Nurse Practitioner with 1+ year of primary care experience.
• Strong clinical assessment, documentation, and patient management skills.

Responsibilities

• Perform comprehensive health assessments and physical examinations.
• Review medications and related instructions; evaluate home safety and psychosocial needs.
• Fulfill documentation for STAR and HCC measures; maintain accurate chart notes on the day of visit.
• Document patient information in charts and case management notes; consider end-of-life preferences and drivers of preventable hospital admissions.
• Counsel patients, caregivers and families; provide instructions for follow-up and care.
• Prescribe or order procedures, treatments, and medications within scope of practice.
