Job summary

Altais Medical Group Riverside in Ohio is seeking a CA-licensed Nurse Practitioner with 1+ year of primary care experience to join a collaborative, purpose-driven team. You will perform patient history and exams, review medications, and support STAR and HCC measures in a patient-centered, team-based environment.

• Universal precautions in patient care
• Prescribe/order procedures, treatments and medications within scope of practice

Responsibilities

• Obtain history and perform physical examination
• Complete review of medicines and related instructions
• Review home safety and psychosocial needs
• Fulfill documentation needs for STAR and HCC measures
• Obtain and document all pertinent information in patient's charts and case management notes on same day as patient is seen; include medications and end of life preferences and "drivers" of preventable hospital admissions
• Instruct all patients, caregivers and family to call you prior to accessing medical care, subject to life threatening emergencies
• Maintain patient safety and confidentiality
• Universal precautions utilized in all patient care
• Prescribe/order procedures, treatments and medications within scope of practice
• Develop high level of trust and intimacy with patients and families

The Base Salary for this position is $135,000 - $155,000/year

Physical Requirements

Office Environment - roles involving part to full time schedule in Office Environment. Based in our physical offices and work from home office/deskwork – Activity level: Sedentary, frequency most of workday. External hires must pass a background check/drug screen.
