La Clínica de La Raza is a community-based health center committed to providing culturally appropriate, high-quality, and accessible health care to the diverse communities of the San Francisco Bay Area. We have spent over 40 years advocating for and creating a health home for those denied access to care. As health care activists, we aim to ensure individuals without health coverage receive the same quality healthcare as insured individuals. Since our founding in 1971, we have recognized that addressing economic and social factors is essential to healing communities. We often extend services into other settings to connect individuals to our health center for preventive and primary care. Over 90,000 individuals visit La Clínica because it is a welcoming space that addresses the whole person, connecting them to a broad network of services to improve their health and well-being. While rooted in activism and social justice, we have grown into a sophisticated provider of primary health care services with 35 sites across Alameda, Contra Costa, and Solano counties.

At La Clínica, you will provide primary care in an outpatient setting, practicing comprehensive medical care. You will serve a resilient, ethnically diverse, and medically complex patient population, aiming for optimal health outcomes. We support our providers through mentorship, quality initiatives, preceptorships, and continued education, including a Nurse Practitioner incubator program for new graduates. Major Areas of Responsibility include but are not limited to:
Healthcare Delivery
  • Provide medical triage, care management, and patient education for a panel of patients.
  • Make referrals to specialists and follow up on bio-psychosocial needs.
  • Deliver evidence-based care to improve health outcomes, patient experience, and resource use.
  • Perform routine, acute, urgent, preventive, curative, and rehabilitative care, determining appropriate regimens.
  • Participate in a cross-functional team for routine and preventive primary care, chronic illness management, urgent care, and triage.
  • Prescribe medications, treatments, therapies, and order diagnostic tests.
Patient Experience
  • Provide culturally humble care for diverse patients of all ages.
  • Participate in evening and Saturday clinics as needed.
  • Build positive rapport with patients and families.
  • Conduct in-home assessments when appropriate.
Collaboration & Mentorship
  • Join a team of providers managing complex cases, reviewing charts, co-signing, and participating in case conferences.
  • Consult with physicians on case management and treatment per protocol.
  • Suggest improvements in clinic flow, scheduling, and service delivery, collaborating with team members including providers, health educators, nurses, and case managers.
  • Supervise medical assistants and provide patient consultation as needed.
Minimum Job Requirements
Knowledge
  • Experience with EHR systems like NextGen or Epic.
  • Interpersonal skills with staff and patients, and knowledge of evidence-based care.
  • Cultural competence in diverse/low-income communities.
  • Familiarity with Electronic Panel Management.
Abilities
  • Dealing courteously and effectively with complex patient profiles and maintaining good relationships.
  • Physical stamina for heavy lifting (up to 50 lbs).
  • Working effectively as part of a team and independently, exercising good judgment, communicating clearly, and working efficiently with minimal supervision.
  • Working under pressure and supporting a multidisciplinary team.
  • Flexibility and adaptability to change.
  • Support for diversity, equity, and inclusion.
  • Accurate record-keeping.
  • Excellent time management and evidence-based approaches to care.
  • English/Spanish proficiency preferred but not required.
Experience And Certifications
  • Current BLS certification (30-day grace period).
  • Board certification required; if not yet certified, must obtain within 6 months.
  • Valid California license and DEA registration.
  • Nurse Practitioner: Current California RN license, national certification (e.g., AANP), and completion of a Nurse Practitioner program with at least 2 years of clinical experience.
  • Physician Assistant: Current California PA license, graduation from an approved primary care program, passing the certification exam, and at least 2 years of full-time clinical experience in family medicine or geriatrics.
