The Alignment Health Advanced Practice Clinician will be part of a program that is designed to provide care and support to our patients by providing an additional level of medical and social support in the comfort of their own home including those with complex and chronic needs.
The APC works within an interdisciplinary team environment that includes physicians, nurse practitioners, physician assistants, case managers, and other health care team members to ensure proper delivery of clinical and home-based patient care.
They assess, develop, and coordinate options and services promoting quality care to achieve optimal health care outcomes while also ensuring cost-effective care complying with Alignment policy and all state and federal regulations.
Our members are typically aged 65 and over with varying acuity levels. Home and virtual visits will be between 45 and 60 minutes depending on the clinical program, acuity level of the member and if it’s an initial or annual visit, or if the member requires an additional level of support for medical and social needs.
