Job summary

Integra Vascular provides patient-focused, preventative vascular care to communities and long-term residents of medical facilities nationwide. Our mission is to minimize wounds, hospitalizations, and patient travel through early diagnosis and exceptional onsite care.

We seek a Nurse Practitioner or Physician Assistant to perform on-site screening, conduct comprehensive assessments, order tests, and collaborate with physicians to deliver tailored vascular care.

Qualifications

• Current Nurse Practitioner/Physician Assistant license.
• Degree from an accredited Nurse Practitioner/Physician Assistant program.
• ACLS
• Current infection control certification.
• Local travel may be required
• PACS and EMR experience preferred
• Clean driving record and personally owned vehicle required

Responsibilities

• Perform on-site screening assessments to determine patient treatment plans at client facilities.
• Conduct age-appropriate history and physical for complex perioperative patients.
• Order and interpret diagnostic and therapeutic tests as needed.
• Assist with care in accordance with facility, state, and federal regulations.
• Monitor the effectiveness of interventions.
• Educate family members, clients, and prospective accounts about services and programs.
• Develop and maintain physician relations as assigned.
• Complete weekly and monthly reports in a timely and comprehensive manner.
• Perform other duties as assigned by Medical Director

Responsibilities:

• Performs on-site screening clinical assessments to determine appropriate patient treatment plans of residents at client facilities by the completion of a comprehensive assessment
• Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill perioperative patients
• Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs
• Assists with the provision of care in accordance with facility, state, and federal regulations
• Monitors the effectiveness of interventions
• Maintains relationships with all appropriate contacts in facilities
• Educates family members, clients, and prospective accounts regarding services and programs
• Assists with the development and maintenance of physician relations, as assigned
• Completes weekly, monthly, and other reports, as assigned, in a timely and comprehensive manner
• Other Duties as assigned by Medical Director
