Job summary

Total Extended Care Services in Michigan is seeking a Nurse Practitioner or Physician Assistant to join our PM&R team in sub-acute rehab settings. You will partner with PM&R physicians and work across interdisciplinary teams to deliver rehabilitative care at skilled nursing facilities.

We offer flexible scheduling with both full-time and part-time positions, on-site care coordination, and a supportive culture focused on improving patient outcomes.

Qualifications

• Licensed NP or PA with MI licensure.
• Experience working in PM&R or sub-acute rehab is a plus.
• Ability to collaborate with physicians, therapists, nurses and staff.

Responsibilities

• Partner with PM&R physician to provide rehab care in sub-acute facilities.
• Coordinate with the primary care and therapy teams for optimal patient outcomes.
• Maintain timely charting and documentation as required.

In this role, you will partner with our PM&R physician in the sub-acute rehab setting.
