Job summary

Total Extended Care Services in Ohio is seeking a dedicated Nurse Practitioner or Physician Assistant to join our PM&R team in sub-acute rehab settings. You will partner with PM&R physicians and an interdisciplinary team to deliver high-quality rehab care at skilled nursing facilities.

This in-person, full-time or part-time role focuses on improving patient outcomes, coordinating care with therapy and nursing staff, and providing consistent, evidence-based rehab services in senior living

Qualifications

• Must hold OH license and be board-certified in their respective role.
• Ability to collaborate with PM&R physicians and interdisciplinary teams.
• In-person care in skilled nursing facilities with adherence to PM&R protocols.

• Partner with the facility's therapy department and primary care team to deliver rehab care; ensure timely charting and documentation.

In this role, you will partner with our PM&R physician in the sub-acute rehab setting. Responsibilities
