Job summary

Total Extended Care Services, a growing Physical Medicine and Rehab group, seeks a dedicated Nurse Practitioner or Physician Assistant to join our PM&R team in sub-acute rehab settings. You will partner with our PM&R physician to improve patient outcomes within an interdisciplinary, specialized team.

This role offers a flexible, part-time schedule, in-person work at skilled nursing facilities, and a collaborative culture with no on-call requirements; timely charting can be done from home.

Qualifications

• Licensed Nurse Practitioner required.
• Licensed Physician Assistant required.
• Certified Nurse Practitioner required.
• Certified Physician Assistant required.
• Must be licensed in MI.
• Reliable transportation.

Responsibilities

• Partner with the facility's therapy department to provide rehab care.
• Collaborate with the patient’s primary care team to ensure outcomes.
• Timely charting/documentation; can be done from home.
