Job summary

Total Extended Care Services is seeking a Nurse Practitioner or Physician Assistant to join our PM&R team in sub-acute rehab settings. You will collaborate with PM&R physicians, therapists, and other clinicians to optimize patient outcomes in skilled nursing facilities.

We offer flexible full- or part-time schedules, on-site care, and a supportive, interdisciplinary environment focused on improving patient function and reducing readmissions. Licensure in OH is required.

Qualifications

• OH-licensed NP or PA with active credentials in PM&R or rehab.
• Board certification in PM&R is a plus.
• Must be licensed in Ohio and in good standing.

Responsibilities

• Partner with the facility's therapy department to provide rehab care.
• Collaborate with the patient's primary care team to ensure optimal outcomes.
• Timely charting/documentation; work can be done from home when appropriate.

In this role, you will partner with our PM&R physician in the sub-acute rehab setting.
