Job summary

Alteas Health is seeking a Nurse Practitioner or Physician Assistant to deliver specialty Pulmonology and Cardiology care in Skilled Nursing, Long-Term Care, and Post-Acute Care settings across Blue Island, Tinley Park, Oak Lawn, and surrounding communities.

You will work with physicians and interdisciplinary teams, travel daily within the territory, and maintain high-quality patient-centered care with a focus on autonomy and collaboration.

Qualifications

• Current, unrestricted Nurse Practitioner or Physician Assistant license in the applicable state(s) of practice.
• Current national certification appropriate to licensure.
• Current Basic Life Support (BLS) certification.
• Minimum six (6) months of NP/PA clinical experience.
• Valid driver's license with reliable transportation.
• Ability to travel daily throughout an assigned territory.
• Strong clinical judgment, communication, organizational, and critical thinking skills.
• Experience utilizing EMR systems.

Responsibilities

• Evaluate, diagnose, and manage acute, chronic, and episodic cardiovascular or pulmonary conditions.
• Develop individualized treatment plans using evidence-based clinical practice.
• Order, interpret, and communicate laboratory studies, imaging, and other diagnostic testing.
• Prescribe medications and treatments within your scope of practice.
• Educate patients, families, and caregivers regarding diagnoses, treatment plans, medications, and ongoing care.
• Coordinate care with physicians, nursing staff, therapy, pharmacy, social services, and other healthcare professionals.
• Maintain accurate, timely, and compliant clinical documentation.
• Round at assigned Skilled Nursing, Long-Term Care, and Post-Acute Care facilities.
• Build strong relationships with facility leadership and clinical teams.
• Represent Alteas Health with professionalism, integrity, compassion, and clinical excellence.

Assigned Territory

This position serves patients throughout the Tinley Park area, providing specialty care at Skilled Nursing, Long-Term Care, and Post-Acute Care facilities located in Blue Island, Tinley Park, Oak Lawn, and surrounding communities. Daily travel within the assigned territory is expected to support patient care and operational needs.

Providers enjoy a full-time, Monday through Friday daytime schedule with no regularly scheduled weekends or traditional on-call responsibilities, allowing for an exceptional work-life balance.

What You'll Do

What We're Looking For

• Experience caring for medically complex patient populations.
