Job summary

Alteas Health is seeking a Nurse Practitioner or Physician Assistant to deliver specialty Pulmonology services in Skilled Nursing, Long-Term Care, and Post-Acute Care settings across Blue Island, Tinley Park, Oak Lawn, and nearby communities.

You'll work full-time, Monday through Friday daytime with no regular weekends or on-call requirements, enabling strong work-life balance while collaborating with physicians and interdisciplinary teams.

Qualifications

• Current, unrestricted NP or PA license in applicable state(s).
• Current national certification appropriate to licensure.
• Current Basic Life Support (BLS) certification.
• Minimum six (6) months of NP/PA clinical experience.
• Valid driver's license with reliable transportation.
• Ability to travel daily throughout assigned territory.
• Strong clinical judgment, communication, organization, and critical thinking skills.
• Experience utilizing Electronic Medical Records (EMR).

Responsibilities

• Evaluate, diagnose, and manage acute, chronic, and episodic cardiovascular or pulmonary conditions.
• Develop individualized treatment plans using evidence-based clinical practice.
• Order, interpret, and communicate laboratory studies, imaging, and other diagnostic testing.
• Prescribe medications and treatments within your scope of practice.
• Educate patients, families, and caregivers regarding diagnoses, treatment plans, medications, and ongoing care.
• Coordinate care with physicians, nursing staff, therapy, pharmacy, social services, and other healthcare professionals.
• Maintain accurate, timely, and compliant clinical documentation.
• Round at assigned Skilled Nursing, Long-Term Care, and Post-Acute Care facilities.
• Build strong relationships with facility leadership and clinical teams.
• Represent Alteas Health with professionalism, integrity, compassion, and clinical excellence.

Assigned Territory

This position serves patients throughout the Tinley Park area, providing specialty care at Skilled Nursing, Long-Term Care, and Post-Acute Care facilities located in Blue Island, Tinley Park, Oak Lawn , and surrounding communities. Daily travel within the assigned territory is expected to support patient care and operational needs.

Providers enjoy a full-time, Monday through Friday daytime schedule with no regularly scheduled weekends or traditional on-call responsibilities, allowing for an exceptional work-life balance.

What You'll Do

What We're Looking For

Required Qualifications

• Experience caring for medically complex patient populations.
