Job summary

Alteas Health is seeking Nurse Practitioners or Physician Assistants to deliver specialized Cardiology or Pulmonology care across Skilled Nursing, Long-Term Care, and Post-Acute Care facilities in Champaign and surrounding communities. The role offers clinical autonomy with physician collaboration and meaningful patient relationships.

The position involves daily travel within the designated territory, weekday daytime hours, and no regular weekends.

Qualifications

• Current, unrestricted NP or PA license in the applicable state(s).
• Current national certification appropriate to licensure.
• Current Basic Life Support (BLS) certification.
• Minimum six (6) months of NP/PA clinical experience.
• Valid driver’s license with reliable transportation.
• Ability to travel daily throughout assigned territory.
• Strong clinical judgment, communication, organizational, and critical thinking skills.
• Experience using Electronic Medical Records (EMR).

Responsibilities

• Evaluate, diagnose, and manage acute, chronic, and episodic cardiovascular or pulmonary conditions.
• Develop individualized treatment plans using evidence-based practice.
• Order, interpret, and communicate laboratory studies, imaging, and other diagnostic testing.
• Prescribe medications and treatments within scope of practice.
• Educate patients, families, and caregivers about diagnoses and plans.
• Coordinate care with physicians, nursing staff, therapy, pharmacy, social services, and other professionals.
• Maintain accurate, timely, and compliant clinical documentation.
• Round at Skilled Nursing, Long-Term Care, and Post-Acute Care facilities.
• Build relationships with facility leadership and clinical teams.
• Represent Alteas Health with professionalism and clinical excellence.

Assigned Territory This position serves patients throughout the Champaign area, providing specialty care at Skilled Nursing, Long-Term Care, and Post-Acute Care facilities located in Champaign, Savoy, Danville, and surrounding communities. Daily travel within the assigned territory is expected to support patient care and operational needs.

Providers enjoy a full-time, Monday through Friday daytime schedule with no regularly scheduled weekends or traditional on-call responsibilities, allowing for an exceptional work-life balance.

What You'll Do As a Nurse Practitioner or Physician Assistant with Alteas Health, you'll provide specialty Cardiology or Pulmonology services while working collaboratively with physicians, facility staff, and interdisciplinary care teams to deliver high-quality, patient-centered care.

Required Qualifications

What We're Looking For

• Experience in Skilled Nursing, Long-Term Care, Post-Acute Care, Internal Medicine, Cardiology, Pulmonology, Hospital Medicine, or Geriatrics.
• Experience caring for medically complex patient populations.
