Clear Destination Inc. seeks full-time Physician Assistants or Nurse Practitioners for a Monday–Friday clinic role in Chula Vista, CA, serving participants of our PACE program.

Under clinical direction of the Medical Director you will provide primary care, perform assessments, develop care plans, and collaborate with an Interdisciplinary Team to meet the needs of frail elders. Applicants must be licensed as PA or NP in California, NCCPA-certified, with at least one year of geriatric experience

Qualifications

  • Graduate of a school of professional physician assistant or nurse practitioner required.
  • Licensed as physician assistant or nurse practitioner in the State of California.
  • Certified as a physician assistant practitioner or nurse practitioner by a national certifying organization recognized by the State of California.
  • A minimum of one year's experience working with the frail and elderly population; two years in geriatric settings desirable.
  • Possession of a valid California License as a Physician Assistant or Nurse Practitioner.
  • DEA Number.

Responsibilities

  • Provide primary care to participants under clinical direction of Medical Director.

Job description

We are looking for full-time Physician Assistants or Nurse Practitioners! This is a Monday - Friday position at our PACE clinic in Chula Vista, CA.

Job Summary

Under the clinical direction and supervision of the Medical Director provides primary care to participants. Performs physical assessments of participants and develops and contributes to implementing appropriate plans of care to St. Paul's PACE program participants. Evaluates participant’s physical complaints and provides appropriate treatment. Provides participant and caregiver teaching and education. In addition to the Primary Care Physician, participates as a member of the Interdisciplinary Team. Demonstrates the knowledge and skills necessary to assess, plan, care for, and provide services to frail elder participants according to assigned responsibilities and PACE standards.

Qualifications and Requirements
  • Graduate of a school of professional physician assistant or nurse practitioner required. Licensed as physician assistant or nurse practitioner in the State of California. Certified as a physician assistant practitioner or nurse practitioner by a national certifying organization recognized by the State of California.
  • A minimum of one year's experiences in working with the frail and elderly population required. Two years experience as a physician assistant or nurse practitioner in a geriatric setting desirable. Shall have either training or related experience in the job assigned.
  • Possession of a valid California License as a Physician Assistant or Nurse Practitioner
  • Degree from a NCCPA certified program as a Physician Assistant or Nurse Practitioner program
  • DEA Number
Skills and Knowledge
  • Thorough knowledge of current concepts, theories, and practices related to home and community-based care for the elderly and disabled adults.
  • Working knowledge of the PACE regulations desired
  • Thorough knowledge of physical, mental and social needs of frail older adults.
  • Ability to lead and work with the interdisciplinary setting
  • Strong organization skills
  • Able to manage changing priorities per needs of the PACE program and the agency
  • At least one year of experience working with a frail or elderly population
