Job summary

Callen-Lorde Community Health Center in Manhattan seeks a Primary Care Provider (NP/MD/DO) with HIV care expertise to join a collaborative, multidisciplinary team for per diem coverage. The role involves patient screenings, diagnoses, and treatments with care personalized to diverse populations.

Expect 20–40 minute appointments, timely follow-ups, and documentation in EPIC, with strong emphasis on patient-centered, respectful care and evidence-based guidelines.

Qualifications

• New York State Certification/Licensure (MD, DO, or NP) required.
• Current NYS Registration and DEA registration certificate required.
• CPR certification required.
• Board certified or eligible in Family Medicine, Internal Medicine, or Infectious Disease preferred.

Responsibilities

• Conduct patient screenings, examinations, diagnoses, and treatments across the lifespan.
• Document all care promptly in EPIC electronic health record.
• Provide primary care services in a per diem capacity.
• Make appropriate internal and external referrals and collaborate with a team-based care model.

Are you a patient-centered healthcare professional committed to delivering high-quality, respectful care to a diverse patient population? Callen-Lorde is seeking a Primary Care Provider (NP/MD/DO) who specializes in HIV care to join our collaborative, multidisciplinary team.

Clinical Care

• Conduct patient screenings, examinations, diagnoses, and treatments across the lifespan.
• Provide care that is respectful, compassionate, unbiased, and consistent with evidence-based guidelines.
• Deliver care in scheduled 20-40 minute appointments and ensure timely follow-up on patient needs, test results, and referrals.
• Document all care promptly in the electronic health record (EPIC).
• Provide primary care services

Collaboration & Coordination

• Make appropriate internal and external referrals.
• Work effectively within a team-based care model, collaborating with other healthcare professionals to ensure coordinated patient care.

Quality & Professional Development

• Participate in quality improvement initiatives and case conferences.
• Engage in ongoing professional development opportunities, including continuing medical education.

• Per Diem Day $400/session
• Per Diem Evening $500/session
• Per Diem Clinic Full Day $750/session
• Per Diem Clinic Half Day $500/session

Work Schedule:

Callen-Lorde is open Monday through Saturday. This is a per diem position and we are seeking 1-2 Saturday coverage.

Qualifications

• New York State Certification/Licensure (MD, DO, or NP)
• Current NYS Registration and DEA Controlled Substances Registration Certificate
• Current CPR Certification
• Board certified or eligible in Family Medicine, Internal Medicine, or Infectious Disease
• Experience in community-based primary care preferred
