Job summary

Anpex Health is seeking a Nurse Practitioner – Palliative & Primary Care to deliver patient-centered care in a collaborative setting in Richmond, VA.

Qualifications

• MSN or DNP NP program completion is required.
• Experience in primary care, geriatrics, or post-acute care is preferred.
• Palliative care training provided.

Responsibilities

• Provide primary and palliative care focusing on symptom management, quality of life, and goal-concordant care.
• Lead and document goals-of-care discussions and advance care planning.
• Facilitate advance directives, POLST/DNR orders, and end-of-life care preferences.
• Manage pain and symptoms using pharmacologic and non-pharmacologic approaches.
• Review vitals and labs and escalate to the attending physician when needed.
• Collaborate with the interdisciplinary team weekly to review patient status and care plans.
• Identify patients suitable for palliative or hospice referral and coordinate transitions of care.
• Support care transitions with emphasis on continuity, comfort, and patient-centered goals.

Education

Master’s (MSN) or Doctorate (DNP) Nurse Practitioner program completion
