Qualifications

• Completion of an MSN or DNP NP program.
• Experience in primary care, geriatrics, or post-acute care preferred.
• Palliative care training and certification provided.

Responsibilities

• Provide primary and palliative care focusing on symptom management, quality of life, and goals-of-care.
• Conduct comprehensive history and physical exams for patients with complex conditions.
• Lead and document goals-of-care discussions, including advance care planning and code status conversations.
• Manage pain and non-pain symptoms using pharmacologic and non-pharmacologic approaches.
• Collaborate with the interdisciplinary team weekly to review patient status and care plans.
• Support care transitions and end-of-life planning.
