Job summary

An Apex Health is seeking a Nurse Practitioner for a hybrid of palliative and primary care to serve patients in the Virginia region. You will deliver symptom-focused care, lead goals-of-care discussions, and coordinate with an interdisciplinary team to improve patient outcomes.

Qualifications

• Active Nurse Practitioner license in Virginia or ability to obtain
• Board-certified in relevant specialty (palliative care or family/adult care)
• Experience in primary or geriatric/palliative settings preferred

Responsibilities

• Provide primary and palliative care focused on symptom management and quality of life
• Lead goals-of-care discussions and advance care planning
• Coordinate care with interdisciplinary team and community resources
• Document care plans and adjust management based on patient goals
• Facilitate transitions of care and hospice/palliative referrals
