Zusammenfassung

Anpex Health is seeking a Nurse Practitioner/Physician Assistant to deliver high-quality primary and palliative care. You will lead goals-of-care discussions, manage symptoms, and coordinate with an interdisciplinary team to align treatment with patient goals and prognosis.

The role emphasizes compassionate care, advance care planning, and timely transitions of care, with supportive leadership and opportunities to grow in palliative and primary care specialties.

Qualifikationen

• Master’s (MSN) or Doctorate (DNP) Nurse Practitioner program completion.
• Experience in primary care, geriatrics, or post-acute care preferred, but not required.
• Palliative care training and certification provided.

Aufgaben

• Provide primary and palliative care services focused on symptom management and quality of life.
• Lead and document goals-of-care discussions including advance care planning.
• Facilitate advance directives and POLST/DNR orders.
• Manage pain and non-pain symptoms with pharmacologic and non-pharmacologic approaches.
• Review vitals and labs and escalate to the attending physician as needed.
• Evaluate and manage new admissions ensuring alignment with patient goals.
• Collaborate with the interdisciplinary team weekly to review patient status.
• Identify patients for palliative or hospice referral and coordinate transitions.
• Support care transitions with emphasis on continuity and patient-centered goals.
• Provide education and emotional support to patients and families.
