Job summary

Anpex Health in Woodbridge, VA seeks a Nurse Practitioner specialized in palliative and primary care to deliver patient-centered care, manage symptoms, and lead goals-of-care discussions. You will coordinate with an interdisciplinary team to ensure smooth transitions and uphold comfort and dignity for patients.

The role emphasizes education for patients and families, advance directives, and participation in care transitions within a mission-driven setting.

Qualifications

• Master’s (MSN) or Doctorate (DNP) Nurse Practitioner program completion.
• Experience in primary care, geriatrics, or post-acute care preferred, but not required.
• Palliative care training and certification provided.

Responsibilities

• Provide primary and palliative care with focus on symptom management and quality of life.
• Lead and document goals-of-care discussions, including advance care planning and code status conversations.
• Facilitate completion and updates of advance directives, POLST/DNR orders, and end-of-life care preferences.
• Manage pain and non-pain symptoms using pharmacologic and non-pharmacologic approaches.
• Review abnormal vitals and labs and escalate to the attending physician as needed.
• Evaluate and manage new admissions ensuring alignment with patient goals and prognosis.
• Collaborate with the interdisciplinary team weekly to review status and care plans.
• Identify patients for palliative or hospice referral and coordinate transitions of care.
• Provide education and emotional support to patients and families regarding prognosis and trajectory.
• Collaborate with nursing, social work, chaplaincy, and therapy services.
