Henry Ford Health seeks an experienced Advanced Practice Provider (Nurse Practitioner) to join its interdisciplinary palliative medicine team in Michigan. The role focuses on comprehensive consultations, care planning, and symptom management for patients with serious illness in the inpatient setting.

Qualified candidates hold a Master's in Nursing, current NP certification, and Michigan RN licensure. BLS/ACLS are optional; APP experience and team collaboration are essential, with CHPN/ACHPN

Qualifications

• Master's degree in nursing is required.
• Current Nurse Practitioner certification is required.
• Current Michigan RN licensure is required.
• BLS/ACLS: not required, but eligible applicants may still apply.
• Experience as an APP (Advanced Practice Provider) is required.

Responsabilités

• Conduct comprehensive assessments addressing physical, psychological, and spiritual concerns for patients with serious illness.
• Lead advance care planning conversations and assist with directive completion.
• Manage pain and symptoms with compassionate, personalized care.
• Collaborate with the interdisciplinary team to discuss goals of care.
• Support emotional wellness and provide health counseling to patients and families.
• Educate clinicians and staff about hospice and palliative care concepts.
• Maintain thorough clinical documentation of patient status and care updates.
• Possibly obtain CHPN or ACHPN certification within 2 years of joining (if NP role).
• Engage in continuing medical education related to palliative medicine.
• Bring APP expertise to the palliative care team.
