AltaMed Health Services Corporation is hiring a Nurse Practitioner, Palliative Care, to deliver comprehensive palliative care to patients in home, facility, and hospital settings. You will conduct visits, coordinate care plans with the care team, and make independent clinical decisions with Medical Director support.

Qualified candidates hold a CA RN license, NP certification (Adult/Family/Pediatric/ Women’s Health), and BLS.

Qualifications

• Graduation from an accredited Baccalaureate school of nursing.
• Current CA RN license.
• Certification as Adult/Family/Pediatric/Women’s Health NP.
• Experience with electronic medical records (EMR).
• Bilingual Spanish and English preferred.
• Valid BLS or higher certification.

Responsibilities

The NP will conduct patient visits for new and established palliative care patients, coordinating their care plans within the designated team. These visits will occur in various settings, including patients' homes, facilities, and hospitals. The NP will also make independent decisions regarding patients' healthcare needs and deliver palliative care services as required. The Medical Director will be available for consultation, collaboration, and referrals as needed.**Minimum Requirements*** Graduation from an accredited Baccalaureate school of nursing.* Completed training in the diagnosis and management of common as well as some complex medical conditions.* Current licensure in the State of California.* Certification as an Adult/Family/Pediatric/Women’s Health Nurse Practitioner.* Previous experience with computer-based systems is required. Electronic Medical records is preferred.* Previous experience in hospice/palliative care, high-risk populations, including home-based care, preferred.* Bilingual in Spanish and English preferred.* A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.**Compensation**Pay for this job starts at $160,680.00 annually**Compensation Disclaimer**Actual salary offers are considered by various factors, including budget, experience, skills, education, licensure and certifications, and other business considerations.
