AltaMed Health Services is seeking a Nurse Practitioner in Palliative Care in the Los Angeles area. The NP will conduct patient visits across home, facility, and hospital settings to deliver high-quality palliative care.

This role requires independent decision-making with collaboration from the Medical Director to optimize patient care. The position emphasizes CA licensure, NP certifications, and experience in hospice/palliative care, with bilingual Spanish/English preferred.

Qualifications

• Graduate from an accredited nursing program.
• Current CA RN license.
• Certification as NP in appropriate areas (Adult/Family/Pediatric/Women’s Health).
• Experience with electronic medical records preferred.
• Hospice/palliative care experience preferred.
• Bilingual Spanish/English preferred.
• Valid BLS or higher per AHA/Red Cross guidelines.

Responsibilities

• Provide comprehensive palliative care visits to patients in homes, facilities, and hospitals.
• Coordinate patient care plans within the care team.
• Make independent clinical decisions and consult Medical Director as needed.

Job Overview

The Nurse Practitioner Palliative Care will provide comprehensive palliative care services to clients, ensuring high-quality and professional care. The NP will conduct patient visits for new and established palliative care patients, coordinating their care plans within the designated team. These visits will occur in various settings, including patients’ homes, facilities, and hospitals. The NP will also make independent decisions regarding patients’ healthcare needs and deliver palliative care services as required. The Medical Director will be available for consultation, collaboration, and referrals as needed.

Minimum Requirements

• Graduation from an accredited Baccalaureate school of nursing.
• Completed training in the diagnosis and management of common as well as some complex medical conditions.
• Current licensure in the State of California.
• Certification as an Adult/Family/Pediatric/Women’s Health Nurse Practitioner.
• Previous experience with computer-based systems is required. Electronic Medical records is preferred.
• Previous experience in hospice/palliative care, high-risk populations, including home-based care, preferred.
• Bilingual in Spanish and English preferred.
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Compensation

Pay for this job starts at $160,680.00 annually
